Description of Services

Premier Mobile Health Services (PMHS) is a 501(c)(3) nonprofit organization providing mobile and walk-in primary healthcare to medically vulnerable and underserved individuals in Southwest Florida. Services include but are not limited to:

  • Wellness exams and preventive screenings
  • Chronic disease management (diabetes, hypertension, and other conditions)
  • Laboratory testing, including lipid panels and basic metabolic profiles
  • HIV testing and STD screening
  • Prescription medication assistance
  • Medical supplies
  • Free medical care for victims of human trafficking, domestic violence, and homelessness
  • Telehealth / virtual visits

PMHS accepts Medicare, Medicaid, and many private insurance plans. Patients without insurance who meet income guidelines (below 200% of the Federal Poverty Guidelines) receive care at no cost. Others are offered care at a significantly reduced sliding-scale fee.

Donation & Refund Policy

2a. Non-Refundable Donations

All donations made to Premier Mobile Health Services are voluntary and non-refundable. Once a donation is submitted and processed, it cannot be reversed, returned, or credited except in cases of documented fraudulent or unauthorized transactions (see Section 2b below).

Because PMHS is a 501(c)(3) tax-exempt organization, all donations are tax-deductible to the fullest extent permitted by law. No goods or services are provided in exchange for donations.

2b. Unauthorized or Erroneous Transactions

If you believe a donation was made in error or as the result of an unauthorized transaction, please contact us within 30 days of the transaction date:

We will investigate the matter promptly. If a transaction is confirmed to be unauthorized or fraudulent, PMHS will work with Bank of America Merchant Services to resolve the issue in accordance with applicable Visa, Mastercard, and American Express card network rules.

2c. Recurring Donations

Donors who enroll in a monthly recurring donation program may cancel their recurring gift at any time by contacting us at info@premiermobilehealth.org or by calling (239) 288-7949 at least 5 business days before the next scheduled processing date. Cancellation will stop future charges but does not entitle the donor to a refund of amounts already processed.

Transaction Currency

All donations and payments processed through the Premier Mobile Health Services website are denominated and charged in United States Dollars (USD). PMHS does not accept foreign currency. If your payment card is issued in a non-USD currency, your card issuer will convert the charge to your local currency; any conversion fees are the responsibility of the cardholder.

PMHS accepts the following payment methods for online donations, processed securely through Bank of America Merchant Services:

  • Visa®
  • Mastercard®
  • American Express® (via American Express OptBlue®)
  • Discover® (where supported)
  • Qualified Charitable Distributions (QCD) from Individual Retirement Accounts (IRAs) — see the Charitable Contribution Form on our Donate page

Card brand marks are displayed at the point of checkout to indicate accepted cards. PMHS does not directly store, process, or transmit credit or debit card numbers on its servers (see Section 8).

Delivery Policy

Premier Mobile Health Services is a nonprofit healthcare provider. We do not sell or ship physical goods. Donations made through this website fund direct patient care, medical supplies, laboratory services, and operational support for our mobile and walk-in clinics.

Healthcare services are delivered in person at our Walk-In Clinic & Administrative Office and at various mobile clinic locations throughout Southwest Florida. Telehealth services are delivered virtually via secure video conferencing. Our current mobile clinic schedule is available at premiermobilehealth.org/mobile-clinic-schedule/.

Tax receipts and acknowledgment letters for qualifying donations are issued electronically via email, typically within 5-7 business days of the donation being processed.

Legal and Export Restrictions

Premier Mobile Health Services provides healthcare services exclusively within the United States. We do not export goods, services, or funds internationally. Online donations must be submitted by individuals who are legally permitted to make charitable contributions under the laws of their country of residence.

Donations from foreign nationals are subject to applicable U.S. law, including but not limited to IRS regulations governing charitable contributions by non-U.S. persons. PMHS reserves the right to decline or return any donation that may create a legal compliance issue.

All activities of PMHS are conducted in compliance with applicable federal, state, and local laws, including the Florida Solicitation of Contributions Act (Chapter 496, Florida Statutes).

Consumer Data Privacy Policy

7a. Information We Collect

When you donate or contact us through our website, we may collect the following types of information:

  • Name, mailing address, email address, and telephone number
  • Payment information (processed and stored by Bank of America Merchant Services; not retained by PMHS)
  • Donation history and preferences
  • Voluntary demographic information provided during registration or contact

7b. How We Use Your Information

PMHS uses collected information solely for the following purposes:

  • Processing donations and issuing tax receipts
  • Communicating with donors regarding their contributions and our programs
  • Sending newsletters and impact reports (opt-in only; unsubscribe at any time)
  • Complying with legal, regulatory, and tax reporting obligations

7c. Information Sharing

PMHS does not sell, rent, trade, or share your personal information with any third party for marketing purposes. We may share information with:

  • Bank of America Merchant Services, solely to complete and process your transaction
  • Government or regulatory agencies as required by law
  • Professional advisors (attorneys, accountants) bound by confidentiality obligations

7d. HIPAA Notice

Patient health information is governed by a separate Notice of Privacy Practices in accordance with the Health Insurance Portability and Accountability Act (HIPAA). Donor information is maintained separately from patient health records and is not subject to the same disclosure rules.

7e. Your Rights

You may request access to, correction of, or deletion of your personal information at any time by contacting info@premiermobilehealth.org. We will respond within 30 days.

Security Capabilities & Payment Card Data Policy

Premier Mobile Health Services is committed to protecting the security of your financial information. The following measures are in place:

  • Our website uses SSL/TLS encryption (HTTPS) to protect all data transmitted between your browser and our web server.
  • PMHS does not collect, store, or process credit or debit card numbers directly on its servers. All card transactions are processed securely by Bank of America Merchant Services.
  • Bank of America Merchant Services is a certified Payment Card Industry Data Security Standard (PCI DSS) compliant processor, meeting the highest level of security standards required by Visa, Mastercard, and American Express.
  • Sensitive payment card data entered during checkout is transmitted directly to Bank of America Merchant Services secure environment and is never accessible to PMHS staff or systems.

Terms & Conditions of Promotions

From time to time, Premier Mobile Health Services may conduct fundraising campaigns, matching gift drives, or other promotional activities. The following general terms apply to all such promotions:

  • Eligibility requirements, matching ratios, and campaign deadlines will be clearly stated in all promotional communications and on the relevant campaign page of our website.
  • Matching gift campaigns are contingent on available matching funds; once the matching cap is reached, additional donations will still be gratefully accepted but will not be matched.
  • PMHS reserves the right to modify or cancel any promotion at any time without prior notice if circumstances require it.
  • Promotional donations are subject to the same non-refundable donation policy described in Section 2.

If no active promotion is indicated on the Donate page at the time of your gift, no promotional terms apply.
